1 BEFORE THE TEACHER STANDARDS AND PRACTICES COMMISSION 2 OF THE STATE OF OREGON i In the Matter of the ) DEFAULT ORDER OF 5 Teaching License of ) PUBLIC REPRIMAND 6 JULIE ANN GODARZI ) g On July 16, 2012, the Teacher Standards and Practices Commission (Commission) issued a 9 Notice of Opportunity for Hearing to Julie Ann Godarzi (Godarzi) in which the Corrunission charged her 10 with Gross Neglect of Duty. The Notice was sent via U.S. First Class Mail and U.S. Certified Mail ll Receipt 701 1 2000 0001 1292 1414 to the address on file with the Commission. The Notice designated 12 the Commission file as the record for purposes of proving a prima facie case. Godarzi signed for the 13 Certified Mail on June 10, 201 l. The Notice of Opportunity of Hearing, dated July 16, 2012, and signed 14 by Victoria Chamberlain, Executive Director, stated: 15 “IF A REQUEST FOR HEARING IS NOT RECEIVED WITHIN THIS 21-DAY PERIOD, 16 YOUR RIGHT TO A HEARING SHALL BE CONSIDERED WAIVED UNLESS YOUR l7 FAILURE TO REQUEST A HEARING WAS BEYOND YOUR REASONABLE CONTROL. 18 IF YOU DO NOT REQUEST A HEARING OR IF YOU FAIL TO APPEAR AT A HEARING, 19 THE COMMISSION WILL ADOPT AN ORDER OF DEFAULT WHICH MAY INCLUDE 5(1) THE REVOCATION OR SUSPENSION OF YOUR LICENSE OR OTHER DISCIPLINE.” 22 Godarzi did not request a hearing. The Commission, therefore, finds Godarzi to be in default and enters 23 the following findings of fact, conclusions of law and order, based on the files and records of the 24 Commission concerning this matter. 25 FINDINGS 0F FACT 26 l. Godarzi has been licensed as a teacher in Oregon since October 2, 2003. She currently holds an 27 Initial 1 Teaching License issued September 15, 2010, with an endorsement in Multiple Subjects Self- 28 Contained (ECE, ELE), valid through October 2, 2013. During all relevant times, Godarzi was 29 employed by the Portland Public School District. 3O 2. On October 5, 2010, Godarzi submitted a doctor’s note placing her on leave from employment with 31 the Portland Public School District until November 30, 2010. Subsequently, Godarzi submitted 32 additional doctor’s notes extending her medical leave through June 16, 2011. Godarzi was advised 33 that the district had assigned her to teach for the 2011/2012 school year and she needed a doctor’s 34 note to extend her leave or needed to formally resign her employment with the District should she not 35 be able to work the following school year.
