The Notice was sent via U.S. First 12 Class Mail and U.S. Certified Mail Receipt #7001 0320 0005 0740 4775 to the address Mr. 13 Rombach provided to the Commission. The certified mail receipt was signed by C. Rombach 14 and returned to the Commission on March 26, 2003. Gary Rombach did not request a 15 hearing. ‘ The Notice of Opportunity for Hearing, signed by Victoria Chamberlain, Executive 17 Director, stated: 18 If a request for hearing is not reoeived within this 21-day period, your right to 19 hearing shall be considered waived unless your failure to request a hearing was 20 beyond your reasonable control. lf you do not request a hearing or if you fail to 21 appear at a hearing, the Commission will uphold the denial of your application -for 22 an Oregon teaching license. ii Based on the foregoing, the Commission finds Gary Wayne Rombach in default and 25 such default is hereby entered. Based on the records and files of the Commission, the 26 Commission enters the following findings of fact, conclusions of law and order. 27 FINDINGS OF FACT 28 1. Mr. Rombach held a Basic Teaching License with an endorsement in Basic 29 Advanced Mathematics valid through February 12, 2003. On or about March 14, 2003, TSPC received an Application for Educator License '2 Form C-1 signed by Gary M. Rombach along with additional documentation to apply for renewal 3 of his Basic Teaching License.
