Mr. Follett‘s fingerprint cards were 12 rejected by the Oregon State Police or the FBI. Mr. Follett has been notified at least 13 twice by U.S. Certified Mail--Return Receipt Requested and U.S. First Class Mail that l4 new prints are required for resubmission, but he has not responded to the re-requests. 15 16 FINDINGS OF FACT l7 18 l. Brian Keith Follett applied for renewal of his teaching license and was issued a 19 Standard License with Physical Education and Health endorsements pending 20 clearance of fingerprints by the Oregon State Police and FBI. 21 22 2. TSPC informed Mr. Follett by U.S. Certified Mail--Return Receipt Requested and 23 U.S. First Class Mail that his fingerprint cards had been rejected. The 24 Commission provided at least 21 days for the applicant to respond. 25 26 3. Mr. Follett has not responded to TSPC notices requesting resubmission of 1 27 fingerprint cards. 28 29 CONCLUSIONS OF LAW 30 31 Refusal to be fingerprinted is grounds for denial of licensure under ORS 342.223(2). 32 33 ORDER 34 35 The Commission suspends the Standard Teaching License #272968, issued to Brian 36 Keith Follett, , for failure to comply with fingerprint requirement.
