The Notice was sent via U.S. 11 First Class Mail and U.S. Certified Mail Receipt 7001 0320 0005 0740 5352 to the address 12 Ms. Danielson provided to the Commission on her application for licensuret The certified mail 13 receipt was signed and dated on January 18, 2003, The Notice of Opportunity of Hearing, 14 dated January 16. 2003, and signed by Victoria Chamberlain, Executive Director, stated: 15 "You, Sandra Jean Danielson, are entitled to a hearing on the proposed action of 16 the Commission. If you want a hearing, you must file a written request for a hearing with the Commission within 21 days of the date of this notice. Attached ‘ to this notice is a copy of the procedures, right of representation and other rights 9 of parties relating to the conduct of a hearing as required under ORS 20 183.413(2)." 21 22 Ms. Danielson did not request a hearing. The Commission, therefore, finds 23 Ms. Danielson to be in default and enters the following findings of fact, conclusions of 24 law and order, based on the files and records of the Commission concerning this matter. 25 FINDINGS 0F FACT 26 1. Ms. Danielson holds a Standard Teaching License with endorsements in Basic 27 Language Arts, Basic Reading, and Standard Handicapped Learner I, issued August 14, 2000, 28 valid until February 24, 2006. 29 2. Ms. Danielson was employed by Oregon City Public Schools as a Special 30 Education Teacher from August 29, 1994 to June 13, 2002. I PAGE 1 — DEFAULT ORDER OF LICENSE REVOCATION - DANIELSON ‘ 3. On June 14, 2002, Oregon City Public Schools reported that during the 2001 -2002 2 school year, Ms. Danielson forged over thirty (30) signatures on twenty-two (22) separate 3 documents. These documents included referral forms, IEP eligibility statements and placement 4 determination forms. 5 CONCLUSIONS OF LAW 6 The above conduct constitutes Gross Neglect of Duty in violation of OAR 584-020- 7 0040(4)(o) and 584-020-0040(4)(c). This conduct constitutes Gross Neglect of Duty.
