10 On July 18, 2001, the Teacher Standards and Practices Commission issued a Notice
ll of Opportunity for Hearing to Maven Klate Huffinan, in which the Commission proposed to
12 suspend or revoke the teaching license of Maven Huffman or otherwise impose discipline
13 under ORS 342.177. The notice was sent via U.S. First Class Mail and U.S. Certified Mail
14 Receipt #7000 0600 0023 8496 8030 to Huffinan’s Beaverton, Oregon address which was the
15 address Huffman provided to the Commission. The certified mail receipt was signed and
16 dated on July 21, 2001, by “SB”and returned to the Commission on July 23, 2001. The
17 notice was also sent via U.S. First Class Mail and U.S. Certified Mail Receipt #7000 0600
18 0023 8496 8047 to Maven Huffinan’s former address in Virginia. This certified mail receipt
1 19 was signed and dated on August 3, 2001, by Louise Morgan, and returned to the Commission
V 20 on August 10"‘. Maven Hufiinan did not request a hearing.

22 The Notice of Opportunity for Hearing, signed by David V. Myton, Executive
1 23 Director, stated:
24 If a request for hearing is not received within this 21-day period, your right t0
25 hearing shall be considered waived unless your failure to request a hearing was
1 26 beyond your reasonable control. If you do no request a hearing or if you fail to 1
i 27 appear at a hearing, the Commission will adopt an Order of Default which may t
28 include the revocation or suspension of your license or other discipline. i

30 Based on the foregoing, the Commission finds Maven Huffman in default and such
31 default is hereby entered. Based on the records and files of the Commission, the
32 Commission enters the following findings of fact and order. ‘

3 l. Maven Klate Huffman holds a Transitional Teacher License with endorsements in i
4 Elementary Authorization Level, Middle Level Authorization and Multiple Subject Self— i
5 Contained valid August 24, 1999 through August 24, 2002. i
6 2. Maven Huffman was employed by the Tigard-Tualatin School District as a sixth i
7 grade teacher in Twality Middle School’ from September 1999 until his resignation v
8 effective March 9, 2001.
9 3. On or about May 22, 1999, Maven Huffinan provided alcoholic beverages to
10 Tigard'Tualatin High School students.
, 11 4. During and subsequent to the 1999-2000 academic year, Maven Huffman dated a
i 12 female high school senior SS. Huffman failed to maintain an appropriate professional i
i 13 student-teacher relationship with SS.

15 CONCLUSIONS OF LAW

17 Maven Huffman’s actions constitute gross neglect of duty and gross unfitness in ‘
18 violation of OAR 584-020-0010(5), 584-020-0035(1)(c)(A) and 584-020-0040(5)(e).

1 22 The Commission revokes the Oregon Transitional Teacher License #307477, issued I
23 to Maven Klate Huffman, for gross neglect of duty and
24 gross unfitness.
