The Notice was sent via UIS. First Class Mail and U.S. Certified Mail 13 Receipt #7003 2260 0000 8804 5900 to the address Ms. Cockrell provided to the 14 Commission. The certified mail receipt was signed by Nancy Santangelo and returned 15 to the Commission on March 17, 2004. The Notice of Opportunity of Hearing, dated ’ March 16, 2004, and signed by Victoria Chamberlain, Executive Director, stated: 17 "You, Anne Marie Cockrell, are entitled to a hearing on the proposed 18 action of the Commission. If you want a hearing, you must file a written 19 request for a hearing with the Commission within 21 days of the date of 20 this notice. Attached to this notice is a copy of the procedures, right of 21 representation and other rights of parties relating to the conduct of a 22 hearing as required under ORS 183.413(2)." 23 24 Ms. Cockrell did not request a hearing. The Commission, therefore, finds 25 Ms. Cockrell to be in default and enters the following findings of fact, conclusions 26 of law and order, based on the files and records of the Commission concerning 27 this matter. 28 /// 29 /// 20 /// ' PAGE 1 — DEFAULT ORDER OF LICENSE REVOCATION AND REVOCATION OF RIGHT TO APPLY — COCKRELL, ANNE ‘I FINDINGS OF FACT 2 1. Ms. Cockrell holds a Basic Teaching License issued July 15, 2002, valid until 3 August 7, 2005. 4 2. Ms. Cockrell served as a teacher for the Archdiocese of Portland. 5 3. On January 27, 2004, Anne Marie Cockrell was convicted of Child Neglect 1, e ORS 163.547, and Possession of Controlled Substance 1, ORS 475.9924A, in the 7 Circuit Court of the State of Oregon for the County of Multnomah. 8 CONCLUSIONS OF LAW 9 The crime of Child Neglect 1 as defined in ORS 163.547 is listed in ORS 10 342.143(3). The Commission is required to revoke the Oregon Basic: Teaching License 11 issued to Anne Marie Cockrell and revoke her right to apply for a license pursuant to ‘ ORS 342.175(2). .
