9 On or about May 11, 1999, the Teacher Standards and Practices Commission received
10 information from the Columbia County District Attorney, indicating that Jack Worden pled
ll guilty to one count of Sex Abuse I. 0n September 2, 1999, Worden was convicted of Sex Abuse
12 I in Columbia County Circuit Court. The Executive Director took action to notify Mr. Worden of
13 intent to revoke his right to reinstate his Oregon teaching license, and to inform him of his
14 opportunity for hearing. The notice dated September 22, 1999, and signed by David V. Myton,
15 Executive Director, notified Worden he had 21 days to request a hearing from the date of the
16 notice. The notice of opportunity for hearing was sent by U.S. Certified Mail, Return Receipt
l7 Requested #2441 250 146 to Worden at the Oregon State Penitentiary. Worden did not request
18 a hearing.

20 FINDINGS OF FACT

22 l. On May 1 1, 1999, Jack LeRoy Worden was pled guilty to one count of Sex Abuse I in
23 Columbia County Circuit Court.

25 2. On September 2, 1999, Worden was convicted of one count of Sex Abuse I, a Class B
26 felony, as defined in ORS 163.4267,in Columbia County Circuit Court

29 ULTIMATE FINDINGS OF FACT

31 Jack LeRoy Worden pled guilty and was convicted of one count of Sex Abuse I, a Class B
32 felony.

34 CONCLUSIONS OF LAW

36 Under ORS 342.175 conviction for the crime of Sex Abuse I requires the Commission to revoke
37 the license of an educator.

41 The Commission revokes the right to reinstate the Oregon Teaching License #225995, issued to
42 Jack LeRoy Worden,
