The Executive Director took action to notify Mr. Worden of 13 intent to revoke his right to reinstate his Oregon teaching license, and to inform him of his 14 opportunity for hearing. The notice dated September 22, 1999, and signed by David V. Myton, 15 Executive Director, notified Worden he had 21 days to request a hearing from the date of the 16 notice. The notice of opportunity for hearing was sent by U.S. Certified Mail, Return Receipt l7 Requested #2441 250 146 to Worden at the Oregon State Penitentiary. Worden did not request 18 a hearing. 19 20 FINDINGS OF FACT 21 22 l. On May 1 1, 1999, Jack LeRoy Worden was pled guilty to one count of Sex Abuse I in 23 Columbia County Circuit Court. 24 25 2.
