1 BEFORE THE TEACHER STANDARDS AND PRACTICES COMMISSION 2 OF THE STATE OF OREGON 3 4 5 In the Matter of the l 6 Teaching License of | DEFAULT ORDER 7 COLETTE DIANE KORETSKY | OF REVOCATION 8 | 9 10 11 On March 21, 2002, the Teacher Standards and Practices Commission issued a Notice l2 of Opportunity for Hearing to Colette Diane Koretsky, in which the Commission proposed to 13 suspend or revoke the teaching license of Colette Koretsky or otherwise impose discipline 14 under ORS 342.177. The notice was sent via U.S. First Class Mail and U.S. Certified Mail 15 Receipt #7000 0600 0023 8496 9686 to the address Ms. Koretsky provided to the 16 Commission. The certified mailing was returned to the Commission as “Undeliverable as 17 Addressed, Unable to Forward”. The notice sent via U.S. First Class Mail has not been 18 returned. Colette Diane Koretsky did not request a hearing. 19 20 The Notice of Opportunity for Hearing, signed by David V. Myton, Executive 21 Director, stated: 22 If a request for hearing is not received within this 21-day period, your right to 23 hearing shall be considered waived unless your failure to request a hearing was 24 beyond your reasonable control. If you do no request a heating or if you fail to 25 appear at a hearing, the Commission will adopt an Order of Default which may 26 include the revocation or suspension of your license or other discipline. 27 28 Based on the foregoing, the Commission finds Colette Diane Koretsky in default and 29 such default is hereby entered. Based on the records and files of the Commission, the 30 Commission enters the following findings of fact and order. 3 1 32 FINDINGS 0F FACT 33 l. Colette Diane Koretsky was employed as a Substitute Teacher at Salem-Keizer 2 Public Schools. 3 3. On October 12, 1995, Colette Diane Koretsky was convicted of Theft II in the 4 Municipal Court for Corvallis, Oregon. 5 4. On December 27, 2001, Colette Diane Koretsky was convicted of DUII in Linn 6 County Circuit Court, Albany, Oregon and sentenced to 24 months probation. 7 5. On December 27, 2001, Colette Diane Koretsky was convicted for the felony 8 crime of Manufacture of Controlled Substance-meth in Linn County Circuit Court, Albany, 9 Oregon. Koretsky was sentenced to 24 months probation, required to pay a money 10 judgment of $2165, undergo mental health evaluation and comply with any recommended 11 treatment. 12 6. On December 27, 2001, Colette Diane Koretsky was convicted of Stalking in 13 Linn County Circuit Court, Albany, Oregon.
