The Notice was sent via U.S. First 11 Class Mail and U.S. Certified Mail Receipt 7006 2760 0000 1638 5568 to the address on file with the 12 Commission. The Certified MaiI receipt was signed for on August 22, 2008, by Mr. Andrew 13 ‘ Humphreys. The Notice of Opportunity of Hearing, dated August 21, 2008, and signed by Victoria 14 Chamberlain, Executive Director, stated: 15 "You, Andrew Humphreys’, are entitled to a hearing on the proposed action of 16 the Commission. If you want a hearing, you must file a written request for a hearing 17 with the Commission within 21 days of the date of this notice. Attached to this notice is 18 a copy of the procedures, right of representation and other rights of parties relating to 19 the conduct of a hearing as required under ORS 183.413(2).” 2(1) Mr. Humphreys did not request a hearing. The Commission, therefore, finds Mr. Humphreys to 22 be in default and enters the following findings of fact, conclusions of law and order, based on 23 the files and records of the Commission concerning this matter. 24 FINDINGS OF FACT 25 1. Mr. Humphrey’s was issued an Initial 1 Teaching License on September 16, 2004, valid 26 until April 7, 2008 with a Multiple Subjects-Self Contained endorsement. 27 2. On July 18, 2008, in the Circuit Court of the State of Oregon for Douglas County, you pled 28 guilty, and were convicted of Contributing to the SexuaI Delinquency of a Minor.
